Old Jefferson Community Care Center is a skilled
nursing and rehabilitation center located in Baton Rouge,
Louisiana, owned and operated by CommCare Corporation.
Our staff specializes in providing skilled nursing, long term
care, and short term rehabilitation such as physical, speech,
occupational and restorative therapies. We serve Baton Rouge and
the surrounding areas. Our state of the art facility works closely
with other regional medical facilities, including Our Lady of the
Lake, Baton Rouge General, and Oschner Hospital with the transition
to nursing homes.
