at Old Jefferson Community Care Center in which residents and
families can participate.
Recreational & Therapeutic Activities
A wide array of activities geared towards various interests and
functional levels are offered at Old Jefferson Community Care
Center. We offer trips out of the facility to restaurants, shopping
and other sites in the area. Families are welcome and encouraged to
visit our facility and participate in social activities with their
loved ones.
Religious Services
Non-denominational chapel provides on-site church services for
residents and their families.
Care Plan Conference
Our staff works with residents and their families to develop a
plan of care that meets their specific needs and goals.
Physician/Dental/Vision/Podiatry
We will arrange for trips to a doctor or specialist or help with
on-site visits so that each resident's care is managed according to
his or her needs and preferences.
